• Cancer details (where applicable)
• Pregnancy details (where applicable)
• Pulmonary Embolism details (where applicable)
• Surgical and Medical condition / treaments including the use of thromboprophylaxsis (where applicable)
• VTE treatment (LMWH & Warfarin)
• Long term follow up (~6mths on selected VTE positive patients)
